PROGRAM MANAGER

  • Manages, supervises, coordinates, evaluates and develops client care teams to ensure quality care delivery and appropriate care management within the agency scope of services and policies, state, federal and local laws and Nurse Practice Act.
  • Reports to: Director of Home Care
  • Supervises/Directs: RN Case Managers and RN/LPN Visit Staff.
  • Provides direction to team members and support staff.

QUALIFICATIONS

  • Graduate of an accredited school of professional nursing. BSN and PHN certification preferred.
  • Current license as a Registered Nurse in the state(s) of practice.
  • Minimum of two (2) years nursing experience in an acute hospital setting or equivalent experience with one year of supervisory experience.
  • Excellent verbal and written communication skills and strong interpersonal skills.
  • Knowledge of home care federal and state regulations.
  • Knowledge of reimbursement sources and documentation requirements within home health care.

HOME HEALTH AIDE

  • Works under the direction of the Registered Nurse or therapist and performs services for the client as necessary to maintain the personal comfort of the client.
  • Reports to: Nursing Supervisor

QUALIFICATIONS

  • Must have completed one of the following:  A. A 75-hour training course and competency evaluation or B. A competency evaluation approved by the Commissioner or C. Training in another jurisdiction equivalent to the 75-hour course or D. A training program or competency evaluation program which satisfies the requirements of Medicare for Home Health Aides as provided by the Code of Federal Regulations, Title 42, Section 484.36
  • Have satisfied the following requirements:  A. Meet the requirements of Title XVIII of the Social Security Act for Nursing Assistants in nursing facilities certified for participation in the Medicare program or have successfully completed a Nursing Assistant training program approved by the State of Minnesota; and B. Have had at least twenty (20) hours of supervised practical training or experience performing Home Health Aide tasks in a home setting under the supervision of a Registered Nurse or complete the supervised practical training or experience within one (1) month after beginning work performing Home Health Aide tasks; or C. Prior to June 1, 1993, had completed an approved Home Health Aide training program.
  • Be at least eighteen (18) years of age.
  • Have demonstrated sympathetic attitude toward care of the sick.
  • Possess the ability to speak English and read and write it well enough to follow directions of the Plan of Care and complete necessary documentation.
  • Possess the maturity to deal effectively with the demands of the job.
  • Have demonstrated the ability to work with little supervision and make appropriate judgments.
  • Have demonstrated dependability, tact and the ability to follow orders.
  • Possess and maintain good physical and mental health, including current TB testing.
  • Have U.S. Citizenship or evidence of valid Alien Work Permit.

PROGRAM COORDINATOR

  • Provides leadership and organization to a single residential home site Assisted Living.
  • Provides supervision and coaching to Direct Support Professionals.
  • Manages the Day to Day logistics and operations of the group home.

QUALIFICATIONS

  • At least 1 year of experience working with individuals with disabilities, preferably in a residential setting.

REGISTERED NURSE

  • Provides services to clients in accordance with the state Nurse Practice Act, agency policy and accepted professional standards of practice. 
  • The Registered Nurse provides nursing services utilizing a comprehensive base of nursing theory and nursing process and communicates/documents observations and assessments.
  • Reports to: Client Services Director/Nursing Supervisor

QUALIFICATIONS

  • Graduate of an accredited school of professional nursing. BSN, PHN preferred.
  • Current license as a Registered Nurse in the state(s) of practice.
  • Minimum of one (1) year experience in an acute care setting or equivalent experience.
  • Current valid driver’s license, good driving record and reliable transportation.
  • CPR certification.
  • Strong communication and interpersonal skills
